Click on a form below to access and print.  The medical form is required before an individual can participate in training or competition with GSO.  The volunteer forms are required before individuals are able to volunteer in any way with Special Olympics.  These volunteer forms are sent to SO Ohio and background checks are performed before individuals are certified to volunteer.  Once that is completed, a short online Protective Behaviors training is also required.
